Fair Oaks Day Hospice
Fair Oaks Day Hospice is based at Walsall Palliative Care Centre.
It offers a nurse led service called the Living Well Programme that provides social and medical support for people with palliative care needs. Patients are welcomed once a week between 10am-3pm for a period of 12 weeks or so.
The Living Well Programme focuses on

Management of physical symptoms

Empowering and educating patients and their loved ones

Social, emotional, spiritual and psychological support

Practical support – including things such as planning for the future or getting equipment

Signposting to appropriate services

Identifying your most significant concerns and priorities, providing a range of support to help manage changes and live well
The team work very closely with a wide range of other health care professionals such as Occupation Therapy, Physiotherapy, Lymphoedema, Palliative Clinical Nurse Specialist team, Counselling, Psychology, Complementary Therapy, Pastoral, Spiritual and Religious care.
Patients can be referred by any health care professional if they have mild to moderate symptoms and live in Walsall (or within a 2 mile radius of the border) and have a Walsall GP.
