Community Nursing
The Community Nursing Service covers all residents in Walsall who are registered with a Walsall GP.
The following services are provided:
Co-ordinated care provision and case management for patients with long-term conditions
Planned programmes of nursing intervention and supportive care
Referral and liaison with other agencies to promote maximum independence
Advice and information on a range of health issues.
The service provides nursing care predominantly in patients own homes. Nurses will visit non-housebound patients whose nursing needs indicate it is more appropriate for them to be seen in their own homes.
Some care considered to require the specialist skills of the District Nursing Service such as wound care, are delivered in clinic settings as close to the patients home as possible.
The service operates 24 hours a day, 7 days a week.
Clinics are based across Walsall.
North locality team based at Pinfold Health Centre
East Locality Team based at Blakenall Village Centre
South Locality team based at Jubilee House
West Locality Team based at Darlaston Health Centre
